Individual Tax Terms of Service

Services to be Rendered:

  • Individual Federal Tax Return 2022
  • State Tax Return 2022 (State(s) of Residence/Work)
  • Previous Year Tax Returns, as Needed

Rates/Methods of Payment:

  • Individual Standard Rate: $105 per hour
  • $50 deposit due to reserve client slot, to be deducted from final amount due
  • $50-$200 rush fee may apply due to late transmission of documents (see within)
  • Remainder of invoice due no later than 30 days after filing unless otherwise agreed upon by both parties
  • Payment methods accepted:

Terms of Rate:

  • Information transmission to Carly Crawford by 11:59pm on February 28, 2023, phone/email response to all questions within 48 hours, and payment of nonrefundable deposit guarantees submission before April 15, 2023 filing deadline.
  • Information transmission to Carly Crawford after February 28, 2023, and/or failure to timely respond to questions during the preparation process, may result in the accumulation of rush fees at $50-$100/return. Information transmission on or after March 15, 2023, and/or failure to timely respond to questions during the preparation process, may result in the accumulation of rush fees at $100-$200/return or a required extension at Carly’s discretion.
  • Preparation fees will be charged by the quarter-hour for any time spent preparing returns and reviewing them with the client. General preparation time is 2.5-4.5 hours per return; Carly cannot guarantee a set total price/hours budget before preparation of return due to the individualized nature of tax preparation.
  • A nonrefundable deposit of $50 is due prior to beginning any draft of a return or reviewing or filing a return. Payment may be made via Check, PayPal, Zelle/QuickPay, or Venmo. The remainder owed will be invoiced after filing and is due in full 30 days after filing unless a payment plan is otherwise arranged.
  • Due to the changing nature of the tax code, items may be treated differently during this preparation season than in previous filings. Carly Crawford will make every effort to explain these changes when the client has questions.

Information Transmission Recommendation:

If the client is working with Carly Crawford remotely, it is recommended that the client sends all documents containing sensitive data through Carly’s encrypted client portal, SmartVault. Carly will create an account for you and invite you to SmartVault upon receipt of your deposit and signed Terms of Service. Carly Crawford will not be responsible for the clients’ data should they choose to submit it in a non-secure format.

Waiver and Disclosure:

To protect you (the client), your tax preparer (Carly Crawford) follows accepted ethical procedures as specified by the Internal Revenue Code and/or applicable guidelines governing the conduct of professional tax preparer.

The specified income tax returns are prepared for me and/or my business at my direction by Carly Crawford, tax preparer.

Information Needed for All Parties on Return:

  • Full Legal Name, DOB, SSN
  • Current mailing address and phone number
  • Your filing status as of 12/31/2021 (single, married – filing jointly, married – filing separately, head of household, qualifying
    widow(er)*)
  • Your Driver License or State ID (to verify your identity)
  • Any W2s for tax year 2021 (employee income)
  • Any 1095-A for tax year 2021 (proof of health insurance through marketplace)
  • A copy of the Federal and State Tax Return filed (1040 and all applicable schedules) for tax year 2020 (for comparison and
    reference)

Information Needed for Any Dependents on Return:

  • Full Name, DOB, SSN
  • Proof of identity (birth certificate/passport)
  • Any childcare expenses, including full-year tuition/cost, caretaker/organization name, caretaker/organization address, and caretaker/organization SSN/FEIN

  • Any 1099-MISC for tax year 2021 (independent contractor income)
  • Any business income not reported to you on W2 or 1099-MISC (invoices, cash payments, etc)
  • Any foreign income documents that were not reported to you on W2 or 1099-MISC
  • Any 1099-K for tax year 2021 (online servicer statement)
  • Any Uber/Lyft/AirBNB income statement
  • Any K-1 Partnership Statement for tax year 2021
  • Any 1099-DIV for tax year 2021 (dividend distributions)
  • Any 1099-INT for tax year 2021 (interest income)
  • Any 1099-G for tax year 2021 (state tax refund/unemployment compensation)
  • Any 1099-B for tax year 2021 (stock income)
  • Any supplemental information for 1099-B (cost basis information for stock sale without basis reported on 1099-B)

Business Tax Terms of Service

Services to be Rendered:

  • Federal Partnership Information Return (Form 1065), Federal S-Corporation Information Return (Form 1120S), or Federal Nonprofit Return (Form 990 series)
  • K-1 Forms for all Partners, as applicable
  • Any State Partnership or S-Corp Returns Required (Note: This is not the same as an annual report; if you need assistance filing an annual report, please reach out to Carly separately.)

Rates/Methods of Payment:

  • Partnership Standard Rate: $140 per hour
  • $100 deposit due to reserve client slot, to be deducted from final amount due
  • $50-$200 rush fee may apply due to late transmission of documents (see within)
  • Remainder of invoice due no later than 30 days after filing unless otherwise agreed upon by both parties
  • Payment methods accepted:

Terms of Rate:

  • Information transmission to Carly Crawford with information required to file all necessary 1099-MISC forms for contractors and/or W2 forms for employees must occur on or before January 9, 2022 in order to guarantee submission by January 31, 2022 deadline.Information transmission to Carly Crawford by 11:59pm on Friday, February 4, 2022, phone/email response to all questions within 48 hours, and payment of nonrefundable deposit guarantees submission by March 15, 2022 deadline.Information transmission to Carly Crawford on or after February 5, 2022, and/or failure to timely respond to questions during the preparation process, may result in the accumulation of rush fees at $50-$100/return. Identifying Information

  • For the Business: EIN (Tax ID #), Legal Name of Partnership, Legal Address/Phone of Partnership
  • For Partners: Legal Name, SSN
  • For Contractors:
      • If paid < $600 in 2019: No further information needed, but include their pay in the P&L statement below as contractor expense
      • If paid $600 or more in 2019: Legal Name, Address, SSN/Tax ID # (should be on their W9)
  • P&L Statement (Profit and Loss Statement), showing gross income/expenses/net income. If you don’t have one of these, let me know how you’ve been handling the recordkeeping, and I will help you create one. This sheet shows ALL income from any source, along with ALL expenses, with descriptions so they can be properly categorized. 
  • How much money each partner invested in the business when you started it to create any starting capital.
  • Your official start date of the partnership.
  • Your Articles of Organization and/or Registration with the State. 
  • The amount of free services you gave to anyone (if that applies), as well as any discounts given in dollars.
  • Information on any business assets (does the partnership own anything in terms of furniture, computers, etc?)
  • Your NAICS Code (You can find this information HERE)
